Real annotations, not a picture of them
The marks are written into the PDF as genuine annotation objects — the same kind Acrobat and Preview create. Open the annotated file in any reader and the highlights are highlights: visible, and the sticky notes pop their text on click. Nothing is rasterised; the document's own text stays selectable underneath.
